Repeat Prescriptions
How to Order a Repeat Prescription
Patients may request repeat prescriptions in the following ways:
- In writing
Either post your computer generated list which was sent with your last prescription
or
Print off the repeat prescription request form on the link below and send to the reception at the surgery.

- Online request
You can request a prescription by emailing reception with the specific names of the medication you require to the following email address:

(Please be aware that this email address is not secure and just like sending a normal email. If you have an issue with this please re-order in writing.)
PLEASE REMEMBER TO ALLOW 48 HOURS NOTICE for the prescription to be processed and remember to list the medication you require and state the destination of the prescription e.g whether the prescription will be collected from the surgery or is to be sent to one of the named pharmacies. You can of course send a stamped addressed envelope for the prescription to be posted to you.
Message in a Bottle
The Message in a Bottle Scheme is supported by Help the Aged, Bath & North
East Somerset Council and the Police. The scheme is designed for elderly
patients to keep a record of any medication they are taking, in case Emergency
Services or other Care Agencies need to access their home.
Your details are stored in a clearly labelled plastic contained, which is kept
in your fridge. Stickers to indicate you use the scheme should be displayed on
your window or door.
